Patient Choice app for iPhone and iPad

OpenPlacements Patient Choice application helps patients understand their post-acute care options and make the right decision about which provider to choose.

Patients are shown images, videos, descriptions and ratings, if available, to help them in this decision process. If a patient is interested in a care provider, they can be shortlisted until later when the patient is asked to rank their preferences for this shortlist. This ranking is then fed back into case management or transition software in the hospital and will further guide the patients transition out of the hospital setting.
